Tight underwear can also lead to vaginitis, which is inflammation and pain of the vagina, including bacterial vaginosis. So whether day or night, I would avoid tight synthetic underwear. The interior acts as a barrier, protecting your clothing from vaginal discharge and blood, and also protecting your vulva and vagina from irritation from clothing."

However, the same specialist also approves not wearing underwear to sleep "always and when you don't sleep in very tight pajamas that would defeat the purpose of not wearing our underwear to bed."

In short, as Welsh explains, "if you create a vagina-positive environment at night, then it doesn't matter if it's underwear (breathable and loose-fitting, ideally) or 'commando.' It all comes down to to personal preference and what works for your body.
